We took all the feedback to the Community Mental Health Teams (CMHT) and we offered to help.
We all find acronyms confusing sometimes, so we have translated some common ones for you.
KMPT stands for Kent and Medway NHS & Social Care Partnership Trust. KMPT provides most of our mental health services in Kent & Medway.
CMHT is the Community Mental Health Team. They are part of KMPT. This is what they can and can’t do and how they work with your GP.
1. CHMT’s can make initial medication prescription, but your prescriptions are down to your GP.
2. If a GP wishes to refer you into specialist secondary mental health services, they usually do this via email/letter and not a long referral process.
3. GPs are encouraged to send urgent referrals to Single Point of Access (SPoA) who will triage them first. If the referral needs a 4 or 24hr response, it might be that crisis team are best placed to see you.
4. CMHT have 28 days to see you once you have been referred
5. For routine appointments some CMHTs call you the day before to remind you of their appointment. If you don’t attend your appointment (this is called a DNA or did not attend) then a member of the team will phone you twice before you are discharged. A letter will be sent to you and your GP explaining that you have been discharged.
6. CMHT’s are not permitted to leave voice mail messages on non-personalized voicemails due to GDPR.
7. GP best practice is to be given anti-depressants before being referred to KMPT. If the GP is concerned that there is no or little improvement, then they should call the advice line and speak with consultant; you should notice an improvement in mood within 4-6 weeks usually.
8. You can request a second opinion of a doctor but there Is no time frame on how long this can take.
9. Non-CPA pathway patients will have a Personal Support Plan (PSP) – this should be reviewed every 6 months.
10. Each time a PSP is reviewed it should be done so with you. A copy of the PSP should be sent to you if you want one.
11. For patients on a CPA pathway these are reviewed every 6 months. All patients should be involved in their care plans.
12. If you see a consultant within secondary mental health your care plan will be sent to you in the form of a letter.
